All individuals coming to Delmont for mental health care treatment require a GP’s referral be sent to the designated area prior to their arrival. Once the referral is accepted by a specialist, the patient will be contacted for either admission or an appointment date.
Each referral letter needs to include:
- Patient's name and contact details
- Type of referral: Inpatient admission/291 assessment/Consultancy and ongoing care/Day Programs
- Current presentation
- Previous and current medical & psychiatric treatments
- Current medications
- Medical Conditions
- Private health funds details
- WorkCover/TAC applicable
Acknowledgement of referrals to the sender will occur after the specialist has accepted and initial contact has been made with the patient. We will endeavour to do so within 5 business days.
